You need to take out a gallon of aquarium water and replace it with a gallon of treated water of close to the same temp water and do this at least once a week, twice or more a week would be better. It culd be stress from the water that it coming from not being close to the water conditions that are in your tank especially if you are just adding water as it evaporates. > well water tested great, my supplier said that the ones that she had pulled a dead one that day ( it happens i mean she has thousands)so she gave me a new one and so far so good. my cat knocked the light into the tank and well warmed it up.
